Plantar Foot Exam - Neuroma, Metatarsalgia, and Plantar Fasciitis pain and treatment options

Foot and Ankle Orthopedic Surgeon MD explains pain on the sole, or bottom, or plantar aspect of the foot. Plantar foot pain including: Neuroma (scar tissue within the nerve), Metatarsalgia (pain at the heads of the metatarsals), and Plantar Fasciitis (sometimes incorrectly referred to as Heel Spurs). Treatment include both operative and non-operative options. Neuroma (Morton's Neuroma) problems includes pain at the base of the toes on the bottom of the foot. Neuromas of the foot can cause burning, tingling, and numbness into the toes, including a sensation of a sock wadded up or the feeling of swelling at the base of the toes. Initial neuroma treatment -- shots, anti-inflammatories, "off loading" pad, to take pressure off the neuroma. Surgical neuroma treatment -- removal of neuroma. Metatarsalgia pain occurs on the bottom of the foot, at the heads of the metatarsal bones. An off-loading pad helps take pressure off the bones. Achilles stretching exercises help strengthen the upward motion of the foot which helps relieve pain. Surgical treatment is rare and involves shaving or breaking the metatarsals. Plantar Fasciitis is common, and is NOT caused by heel spurs, although pain is often felt in the heel. Plantar Fasciitis is caused by the tearing of fascia at the heel and is usually caused by a tight achilles tendon. Conservative Plantar Fasciitis treatment includes shots, a cushion insert, an extension boot, a night splint, and exercises to stretch the achilles tendon. Surgical treatment of Plantar Fasciitis involves releasing the tarsal tunnel and releasing part of the plantar fascia.
